This information collection is subject to the Privacy Act of 1974, as amended (U.S.C. Section 552a).  Collection of this information is authorized under 5 U.S.C. Sections 301 and 302, 42 U.S.C. Sections 203 and 282, 44 U.S.C. Section 3101 and E.O. 10450.  The primary use of this information is to document reviews and investigations undertaken by the Office of Management Assessment, NIH to provide management or the Office of Inspector General, Office of the Secretary, HHS with information needed to take action to resolve complaints of misconduct or alleged violations of statutes, regulations, policies, or the terms and conditions of funding.  This information may be disclosed to NIH management and legal counsel, the Office of the Inspector General and personnel offices for disciplinary/separation actions.  Submission of this information is voluntary, however, in order for us to address your allegation, please complete the relevant fields.  Alternatively, you may report an allegation by phone or in writing to the address below, or by calling (301) 496-5586
